⚠️ Please DO NOT cancel your telephone with your existing provider, if you have already requested them to cancel your service please call them urgently and ask to retract the cancellation or you will risk losing your number.
To complete this form, you will need to provide:
- A copy of your latest telephone bill. The bill must show the account holders name, address and telephone number to prove ownership. You can find your telephone bill in the online portal of your existing provider in most cases, look for a "PDF" or "Printable" version for best results.
