• INCIDENT REPORT

  • The Incident Report should be completed by any chapter or colony following an event or incident at which an injury, loss, or other health and safety violation has occurred. This form must be submitted within 24 hours of the incident.

    Once the Incident Report is submitted, our Fraternity Service Center staff members will follow up with your chapter or colony, if necessary, and contact appropriate parties.
