Below you will need to provide the following information:
- The name of the person(s) creating the trust (if that is you, just input your name);
- A person (or persons) who you want as Trustee(s);
- A beneficiary (you can have as many as you'd like);
- A description of the trust propety;*
- Instructions for the Trustee(s) to follow; and
- The name of the trust (you can name your trust whatever you'd like (most people use the name of the beneficiary)).
- Finally, you'll need to choose whether this trust is revocable or irrevocable.
For Example:
"I direct that my trustee shall manage the trust property and shall distribute it to the named beneficiary when said beneficiary attains the age of 18."
*NOTE: If the trust property is real property (i.e. land, home, etc.) you will have to provide the mailing address or the [city], [county], [state], and [zip code] of where the property is located. You will also have to execute a Quit-Claim deed transfering the property to the trust.
