Certificate of Appreciation Order Form

Certificates of Appreciation are to acknowledge special service and achievement to the Fraternity on any level either by a member, non-member or chapter.

Recipients receive a printed certificate denoting the recipient’s service.

There are no requirements for this, and the cost is $5.00 each plus shipping.

Recipients may be nominated by any individual or governing body, with approval and presentation being given by any governing body.

Please Note: Award nominations must be submitted no later than ten days in advance to allow for processing.
